Why it matters for appointment-based businesses
Clients rarely tell you to your face that they weren't happy with a cut, a treatment or a class — they just don't rebook. A short form after the appointment gives them an easy, private way to tell you, and gives you the chance to put it right.
Example questions
Describe your goal as "how was your appointment today?". You might get:
- How happy are you with your result? (1–5 rating)
- How would you rate your stylist/therapist? (1–5 rating)
- How likely are you to book again? (Very likely · Maybe · Unlikely)
- Anything we could do better next time? (open text)
How to share it
- QR code at reception or the mirror station so clients can answer before they leave.
- Share link in your follow-up text or email, or in your booking confirmation.
- On aftercare cards handed out with products.
Make it look like your brand
Add your logo and button colour, and write your own welcome and thank-you messages — for example, thank clients and remind them how to rebook. Gyms and studios can run separate forms per class or instructor and filter results by form.
